1 dead following officer-involved shooting at Relapse Bar & Grill, coroner identifies man dead
COLUMBIA, SC (WOLO) — According to Sheriff Leon Lott, one man is dead following an officer-involved shooting on Sunday morning, May 10, at Relapse Bar & Grill at 5024 Two Notch Road.
Speaking alongside Forest Acres Police Chief Don Robinson, Lott says Corporal Daniel Haddad attempted to stop a vehicle erratically entering the parking lot to the club.
Lott says Haddad exited his vehicle and gave commands for the driver to stop. The driver then accelerated, almost striking workers at the club. Haddad fired his weapon, striking the driver, who died at the scene.
According to Richland County Coroner Naida Rutherford, the victim was identified as 32-year-old Cody Johnson of Lexington.
The shooting occurred within the jurisdiction of RCSD.
According to Lott, RCSD has received 64 calls since November regarding noise complaints, assaults, and fights at the club and also says their liquor license expired in March but they continued serving patrons.
“It would’ve been avoided if they had closed the dadgum doors. If they were not open, and that’s all we’re gonna do right now, in addition to the investigation into the officer-involved shooting we’re gonna do something separate on that place. And I’m gonna do everything I can to make sure they close and never open again. What I saw last night — was absolutely terrible,” says Lott.
Chief Robinson says Corporal Haddad has been placed on administrative leave following an investigation by RCSD.

“We will cooperate fully with the Sheriff’s Department as they investigate this officer involved shooting. At this time, I’ve placed Cpl. Daniel Haddad on administrative leave pending the outcome of this investigation, which is standard protocol. Our thoughts are with all involved,” says Robinson.
Cpl. Daniel Haddad has been employed by FAPD since July 2021.
This is the first deadly officer involved shooting involving a Forest Acres Police Officer in decades.
“Forest Acres is a city rooted in community and faith,” says Mayor Thomas Andrews. “During difficult times like these, we ask for patience as law enforcement investigates. We as a city also affirm our commitment to transparency. Finally, we ask for prayers for all involved, especially families touched by loss.”
